Overview

An exciting opportunity within the Health and Justice Reconnect team for the role of Health and Justice Support, Time, Recovery Worker based in Sussex. The role may include visits to HMP Lewes, HMP Ford and Immigration Removal Centre (IRC) Gatwick as well as supporting Service Users in the community. The base for this role will be the IRC Gatwick. The Health and Justice services identify people who have substance misuse, mental health, learning disability or other vulnerabilities when they first come onto contact with the criminal justice system or Immigration Removal Centre. We look to identify unmet need and vulnerabilities that often lead to or exacerbate offending behaviour, offer onward referral and signposting as appropriate. We liaise across agencies to support onward planning and intervention to address identified needs. The Health and Justice service work in Police Custody Suites, Courts, Prisons, Immigration Removal Centre and Community throughout Sussex and offer assessments to individuals within the Criminal Justice system including Voluntary Attendees. We help to inform decision makers with relevant and timely information to support sentencing and disposal outcomes. This is an all age service and so you will be required to work with people from the age of 10 years upwards.

Responsibilities
  • THIS ROLE IS SUBJECT TO ADDITIONAL VETTING
  • The post holder will be expected to travel across the MPFT footprint. Working with service users referred to Health & Justice and the individual's carers, helping to promote self-determination, choice and autonomy.
  • To effectively establish a therapeutic relationship with service users and relatives / carer.
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Assess patient's individual care needs, agree and refer on to appropriate agency. Involve the individual in the planning and prioritisation of their activity where possible.
  • Provide short term intervention to encourage individual to engage with services and attending their initial appointments.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
  • Utilise agreed outcome measures to review the effectiveness of process, as part of Health & Justice Service.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
  • Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieved.
  • To attend relevant clinical meetings / forums as requested.
  • To plan and manage own delegated workload on a day to day basis.
  • To support with the planning induction of students and new staff.
  • To remain up to date and compliant with mandatory training requirements.
