Town & Country Housing is seeking a Health and Housing Coordinator to support safe, timely discharges and maintain independent living for patients and families in the community.
Based at Tunbridge Wells Hospital in Pembury, this full-time, 15-month fixed-term role is 35 hours per week, with hybrid working and two days in the office. You will coordinate housing solutions, liaise with health and local authorities, and safeguard residents while promoting wellbeing.

This is a full time 15 month fixed term position working 35 hours a week and is based at Tunbridge Wells Hospital in Pembury.
Salary of £29,647

This is a hybrid role where we expect all staff to come into their base office 2 days a week (everyday during induction which could be up to 3 months).
