Job summary

Avicenna Supported Living in Peacehaven seeks an experienced Team Leader to supervise staff and provide hands-on support for adults with learning disabilities and autism.

You will write and review care plans, manage risk assessments and support PBS plans, while ensuring documentation is accurate and up to date. A strong leadership presence and commitment to person-centred support are essential.

Qualifications

• Level 3 NVQ/Diploma in Health & Social Care or equivalent.
• Experience in adult social care, ideally learning disabilities/autism.
• Leadership or supervisory experience.
• Knowledge of Positive Behaviour Support (PBS).

Responsibilities

• Lead and support staff during shifts.
• Write, review, and update care plans and risk assessments.
• Support PBS plan implementation and review.
• Conduct staff supervisions and competency assessments.
• Monitor care records for accuracy and timeliness.
• Assist with medication management, safeguarding and QA.
• Coach staff to maintain good practice.
• Collaborate with management, families and other professionals.
