Job summary

Avicenna Supported Living is seeking an experienced Team Leader to oversee a Supported Living service for adults with learning disabilities, autism and complex needs. You will be hands-on, guiding staff, ensuring high standards of care, and supporting day-to-day service delivery.

The role combines direct support with staff supervision, care planning, risk assessment and quality assurance to promote independence and person-centred outcomes.

Qualifications

• Level 3 NVQ/Diploma in Health & Social Care or equivalent.
• Experience in adult social care, ideally learning disabilities or autism.
• Previous senior, supervisory or leadership experience.
• Knowledge of Positive Behaviour Support (PBS).
• Experience writing or reviewing care plans and risk assessments.
• Understanding safeguarding and person-centred support.
• Ability to coach and develop staff.
• Good written, verbal and IT skills.

Responsibilities

• Lead and support staff during shifts.
• Write, review and update care plans and risk assessments.
• Support PBS plan implementation and review.
• Complete staff supervisions and competency assessments.
• Monitor care records for accuracy.
• Support medication management, safeguarding and QA.
• Coach staff to maintain good practice.
• Collaborate with management, families and professionals.
• Provide direct support when needed and promote independence.
