Job summary

Avicenna Supported Living in Peacehaven, East Sussex, is seeking an experienced Team Leader to manage their Supported Living service, supporting adults with learning disabilities, autism and complex support needs. This is a hands-on leadership role combining direct care with guiding staff and maintaining high standards of care.

You will lead shifts, develop care plans, monitor records and medication management, supervise staff, and collaborate with families and professionals to promote

Qualifications

• Level 3 NVQ/Diploma in Health & Social Care or equivalent.
• Experience in adult social care, ideally learning disabilities and/or autism.
• Senior/supervisory or leadership experience.
• Working knowledge of Positive Behaviour Support (PBS).
• Experience writing or reviewing care/support plans and risk assessments.
• Good safeguarding and person-centred support understanding.
• Confident in supporting, guiding and developing staff.
• Good written, verbal and IT skills.
• Experience completing staff supervisions and competency assessments.
• Experience supporting people with complex needs or behaviours of concern.
• Medication and quality assurance experience.
• Full UK driving licence and access to a vehicle.

Responsibilities

• Lead and support staff during shifts.
• Write, review and update care plans and risk assessments.
• Support the implementation and review of Positive Behaviour Support (PBS) plans.
• Complete staff supervisions and competency assessments.
• Monitor care records and ensure documentation is accurate and up to date.
• Support medication management, safeguarding and quality assurance.
• Coach and guide staff to maintain good practice.
• Work closely with management, families and other professionals.
• Provide direct support when required and promote independence, choice and positive risk-taking.
