Job summary

NHS Jobs is seeking a GP Clinical Lead for the Proactive Care Service in Berkshire West. The role provides senior clinical leadership, ensures governance and safe escalation pathways, and supports the mobilisation and implementation of the service.

Responsibilities include developing SOPs, managing caseload processes, and coordinating with geriatric and primary care colleagues to optimise frailty management, deprescribing, and patient step-down when appropriate.

Qualifications

• GMC standards and PCA governance adherence.
• Management of safeguarding, information governance, confidentiality and clinical safety.
• Maintain professional indemnity / cover for the engagement model.

Responsibilities

• Provide senior clinical leadership for PCS and act as principal clinical lead for the service.
• Lead development and maintenance of clinical SOPs, inclusion/exclusion criteria, caseload processes, escalation pathways and step-down arrangements.
• Oversee frailty assessment, falls, medication optimisation/deprescribing, and CGA; support complex case discussions and escalation routes.
