Job summary

Change Grow Live in Camden is seeking a Prison In-Reach Recovery Coordinator to assess and engage service users, ensuring timely access to the right interventions and continuity of care. You will work across prison and community settings to support transition and recovery.

You will conduct prison-based screenings and risk assessments, develop recovery plans with service users prior to release, and provide harm reduction education and Naloxone training.

Qualifications

• Experience working with individuals with substance misuse or similar fields.
• Excellent verbal and written communication skills; proficient with IT.
• Ability to manage time, take initiative and prioritise tasks.
• Good understanding of mental health interventions and services.
• Strong interpersonal skills and ability to collaborate with diverse professionals.
• Ability to adapt to change while prioritising service users' needs.
• Commitment to information governance and safeguarding requirements.
• Enthusiasm for ongoing learning and staff development.
• Must be approved for DBS and security clearance requirements.

Responsibilities

• Act as a liaison between prison in-reach teams and community services to support continuity of care and coordinated risk reduction planning.
• Carry out prison-based screenings, initial assessments and risk assessments; develop recovery plans with service users prior to release.
• Provide harm reduction education, overdose prevention and relapse management planning with service users in custody and after release.
• Deliver and promote Naloxone provision before and after release, and train service users and peers in its use where appropriate.
• Offer through-the-gate support and facilitate access to housing, health appointments and community recovery services after release.
• Conduct home visits, hostel visits and community outreach to maintain engagement, especially for high-risk individuals.
• Collaborate with police, probation, mental health, housing and social care partners to address complex needs.
