Zusammenfassung

Hampshire Hospitals NHS Foundation Trust is seeking a Gastro Cancer Pathway Navigator within the Medicine Division to act as the central contact for patients on the gastro cancer pathway, ensuring coordinated, timely and patient-centred care throughout their diagnostic journey.

You will work with multidisciplinary teams to support pathway management, identify barriers to care and escalate delays to ensure rapid access to investigations, endoscopy and treatment while upholding Cancer Waiting Time

Qualifikationen

Experience coordinating multidisciplinary clinical pathways.
Excellent organisational and communication skills.

Aufgaben

Coordinate gastroenterology and endoscopy cancer pathways to ensure timely progress through referrals, diagnostics and treatment.
Work with Endoscopy Unit teams, booking offices and diagnostic services to maximise endoscopy capacity and timely access to investigations.
Monitor patients awaiting endoscopic procedures and escalate delays or capacity issues to maintain Cancer Waiting Time standards.
Validate and maintain gastroenterology and endoscopy pathway tracking systems for accurate performance reporting.
Facilitate timely booking of diagnostic tests, endoscopic procedures and follow-up appointments to maintain smooth patient flow.
Contribute to service improvement initiatives to enhance pathway efficiency and patient experience.
