Job summary

The Methodist Church in Britain is advertising a Band 3 Community Care Assistant role in the City District Nursing Team serving Wye Valley NHS Trust. The post involves home visits for observations, venous bloods and wound care, with education and support for patients, and lone working across the locality.

A Diploma/NVQ Level 3 in Care and a CPD diary are required; travel or a vehicle is needed as not all bases have public transport.

Qualifications

• NVQ/Diploma Level 3 in Care.
• CPD diary demonstrating learning and reflection from courses and experience.
• Experience of working in a health, social care or related environment.
• Awareness of health and safety and manual handling requirements.

Responsibilities

• Monitor patients' observations during home visits, alongside band 5 Community Staff Nurses
• Obtain venous blood samples
• Provide wound care
• Provide patient education and support
• Lone working, visiting patients' homes within the locality
