Wye Valley NHS Trust in Hereford is recruiting a part-time Community Support Worker to join the City District Nursing Team. The role involves home visits, supporting district nurses, monitoring observations, and providing wound care and education.

We value caring, respectful team players. Training is available if needed; a vehicle may be required to reach patients' homes and ensure timely care delivery.

Qualificações

• NVQ/Diploma Level 3 in Care is preferred.
• CPD diary showing learning and reflection is expected.

Responsabilidades

• Provide home-based care and support to patients in their own homes.
• Support Band 5 Community Staff Nurses by monitoring observations and basic wound care.
• Travel to patient homes; access to a vehicle may be required for work.
