Job summary

We are looking for an organised administrator to support the Frailty Team within the Primary Care Network in Gosport. You will handle office administration, coordinate appointments and meetings, and maintain health data across systems.

This 6-month fixed-term role involves liaising with patients and external partners, prioritising urgent requests, and helping to keep the frailty pathway running smoothly under the lead frailty nurse and care coordinators.

Job description

We are looking for someone to provide high-quality administrative support to the Frailty Team within the Primary Care Network (PCN), contributing to the delivery of proactive, coordinated care for patients living with frailty.

This role involves supporting multidisciplinary working, managing clinical and non-clinical data, coordinating meetings, liaising with patients and external partners, and ensuring smooth operational processes within the frailty pathway.

You will be a key point of contact for the team and help to ensure efficient day-to-day administration

The role is to carry out office administration, working in support of the lead frailty nurse, acute visiting service and working alongside care coordinators to aid with supporting safe patient care. You will be required to use of a range of computer systems. As a first point of contact for patients, you will identify and prioritise information to inform decisions on the urgency of a response. The ability to work under own initiative and to prioritise own workload and to work to deadlines is essential.

This is a 6 months fixed term contract.
