Job summary

Norfolk & Norwich University Hospitals NHS Foundation Trust is seeking an experienced Health Care Support Worker for the Frailty SDEC/Loddon team. You will provide clinical and therapeutic care, deliver high-quality personal care, and work within a multidisciplinary team to support elderly patients through the frailty pathway.

Role requires significant HCA experience with enhanced frailty skills; flexible to full or part-time shifts at the Norwich area hospital, with NHS benefits and development

Qualifications

Significant experience as Health Care Assistant.
Enhanced skills in frailty care and SDEC/Loddon unit.
Ability to follow protocols and work within a multidisciplinary team.

Responsibilities

Deliver clinical and therapeutic care to patients within the band 3 HCA remit.
Plan and deliver high standards of individualized personal care.
Communicate effectively with patients, families and the team.
Carry out instructions from registered practitioners and undertake delegated clinical skills.
Demonstrate professional responsibilities and uphold PRIDE values.
Work with the multidisciplinary team ensuring good handover and planning for patient journeys.
