Job summary

South Downs Health and Care Ltd in Eastbourne is seeking two experienced Frailty Practitioners to join our developing Integrated Neighbourhood Team. You will assess, manage and coordinate care for frailty patients, working with GPs, Community Geriatricians and a multidisciplinary team to deliver proactive, person-centred care at home.

You will contribute to service development, maintain high governance standards and support integrated frailty pathways across local services.

Qualifications

• Autonomous practitioner with case management experience
• Experience working with older people living with frailty and/or complex conditions
• Ability to work with statutory and non-statutory agencies to support patient-centred care
• Willingness to undertake non-medical prescribing training within 12 months
• Registered with NMC or HCPC and active professional registration

Responsibilities

• Assess and manage frailty patients with CGAs and complex needs
• Develop personalised care plans with patients, carers and MDT
• Coordinate care across community, hospital pathways and boundaries
• Prescribe within professional competencies as a non-medical prescriber
• Lead care coordination and liaison with GPs, nurses and social care
• Support governance, quality improvement and service development
