Job summary

Qualifications

• Expert management of complex frailty syndromes, dementia, delirium, polypharmacy, falls risk and end of life needs.

Responsibilities

• Attend MDT meetings and case reviews.
• Conduct proactive geriatric assessments and care plans.
• Deliver autonomous clinical decisions within scope and prescribing competencies.
• Lead weekly care home reviews and daily acute home visits as required.
• Participate in clinical supervision and CPD activities.
