Promni Health is seeking an Advanced Clinical Practitioner to join a Primary Care Network multidisciplinary team for a six-month, full-time contract based in the host PCN. The post focuses on advanced assessment, diagnosis and prescribing for frail older people living in care homes, to prevent avoidable hospital admissions.

The role involves regular travel between care homes and GP sites across the network and requires Masters in advanced practice, along with registration with the relevant body.

Formación

• Masters degree in advanced practice.
• Competence across the four pillars of advanced clinical practice.
• Evidence of ongoing CPD.

Responsabilidades

• Carry out advanced clinical assessment and management planning for care home residents.
• Prescribe safely in line with national guidance and local pathways.
• Identify deterioration and triage urgent presentations to avoid admissions.
• Lead MDT meetings and care home reviews.
• Support education and development of care home staff.
• Maintain accurate clinical records and governance.
