Chesterfield Royal Hospital NHS Foundation Trust is seeking a fully qualified Advanced Clinical Practitioner to join the acute frailty team. The role focuses on early Comprehensive Geriatric Assessment and optimizing patient care through advanced clinical skills and leadership.

Key duties include independent high-level decision making, education delivery, and involvement in service improvement. This post supports a 7-day rotas pattern with 37.5 hours per week in Chesterfield, UK.

Formación

• Registered healthcare practitioner with relevant professional registration.
• ALS provider or willing to complete certification.
• Aspiring/holding MSc in Advanced Practice or MSc completed in Advanced Clinical Practice.

Responsabilidades

• Provide high-quality clinical assessment and treatment for older adults with frailty.
• Be part of the Acute Frailty multidisciplinary teams and lead care pathways.
• Deliver education, provide clinical leadership and contribute to service development.
