Job summary

South West London and St George's Mental Health NHS Trust is recruiting a Forensic Community Psychiatric Nurse and Care Co-ordinator to manage a caseload of forensic cases, act as social supervisor as appropriate, and work within an MDT. Qualifications

• Current NMC registration and evidence of continuing professional development.
• Experience of working with people with serious mental health problems in a multidisciplinary setting.
• Knowledge of Care Programme Approach and risk assessment in forensic mental health.

Responsibilities

• Manage a defined caseload of service users with forensic histories and coordinate care plans.
• Collaborate with police, MOJ, probation, housing and residential providers to ensure high-quality service delivery.
• Lead on risk assessment, engagement, and interventions to support recovery and goals.
• Maintain high standards of practice and participate in weekly MDT meetings.
