Qualifications

• Registration required: HCPC/NMC or Social Work England (open to HCPC/NMC/Social Work England registered professionals).
• Open to Occupational Therapists, Community Psychiatric Nurses or Social Workers.

Responsibilities

• Manage a caseload of up to 30 patients in a community setting.
• Attend key meetings and contribute to KPI monitoring and service improvement.
• Coordinate delivery of nursing care with MDT to support recovery.
• Lead and supervise junior staff and provide guidance on care delivery.
• Monitor patient observations, safety and medication monitoring within trust policies.
