South West London and St George’s Mental Health NHS Trust seeks a Forensic Community Psychiatric Nurse and Care Co-ordinator based in London. Qualifikationen

• Qualified RMN or Learning Disability Nurse with sound experience in mental health.
• Knowledge and experience of forensic mental health service provision and offender care.
• Current NMC registration and commitment to professional standards.
• Demonstrated ability to work effectively within a multidisciplinary team.
• Up-to-date mandatory training in line with Trust policies and procedures.

Aufgaben

• Manage a defined caseload as care coordinator and social supervisor where appropriate.
• Collaborate with agencies (police, MOJ, probation, housing, providers) to ensure high-quality care.
• Develop and implement care plans, regularly reviewing and updating them.
• Apply psychosocial approaches within a health and psychosocial framework to support recovery.
• Lead on risk assessment and engagement to achieve service-user goals.
• Coordinate admissions, discharges and transfers per Trust policies.
• Communicate effectively with clients, carers and services and provide information on available support.
• Attend weekly clinical meetings to report caseload and progress.
