Job summary

The Community Forensic Outreach Service (FOS) within South West London and St George's Mental Health NHS Trust is seeking a Forensic Community Psychiatric Nurse and Care Co-ordinator. You will manage a caseload of patients with forensic histories and work with an MDT to deliver coordinated care.

Responsibilities include risk assessment, engagement with service users, carers and partner agencies, and ensuring care plans are implemented and reviewed.

Qualifications

• Qualified RMN or Learning Disability Nurse with appropriate NMC registration.
• Experience in mental health and forensic service provision.
• Ability to manage caseloads and coordinate care with a multi-disciplinary team.

Responsibilities

• Manage a defined caseload of service users with forensic histories.
• Coordinate care plans and monitor progress with carers and MDT.
• Perform risk assessment and safety planning.
• Engage with police, probation, housing and other agencies to ensure integrated care.
• Maintain up-to-date knowledge of Trust policies and mandatory training.
