South West London & St George's Mental Health NHS Trust is recruiting a Forensic Community Psychiatric Nurse and Care Co-ordinator to manage a defined caseload of adults with forensic histories. You will act as care co-ordinator and social supervisor within an MDT, providing risk assessment, engagement, and recovery-focused interventions.

The role includes collaboration with police, MOJ, probation, housing and social care, with regular supervision and professional development opportunities.

Qualifications

• Qualified RMN or Learning Disability Nurse with current NMC registration.
• Experience in mental health, especially forensic settings, and offender care.
• Knowledge of Care Programme Approach and risk assessment practices.

Responsabilités

• Manage a caseload of forensic patients and act as care coordinator.
• Lead on engagement and interventions to support recovery and goals.
• Collaborate with police, MOJ, probation, housing, and residential providers.
• Provide medication management where applicable and ensure proper care planning.
• Attend weekly clinical meetings and supervise therapeutic effects of prescribed medications.
