Job summary

The Community Forensic Mental Health team at Cornwall Partnership NHS Foundation Trust is recruiting a Registered Mental Health Nurse to provide care coordination and caseload management for individuals on secure pathways or under the Mental Health Act following criminal justice involvement. Travel across Cornwall and out of county to secure services is central.

Qualifications

• Current NMC registration as a Registered Mental Health Nurse
• Experience in adult community mental health, forensic community, secure or adult inpatient settings
• Experience of managing a caseload
• Evidence of continued professional development
• Sound working knowledge of the Mental Health Act 1983 and other mental health legislation
• Knowledge of Ministry of Justice restriction orders
• Level 1 police vetting

Responsibilities

• Provide care co-ordination and community forensic interventions, including assessment, consultation, care planning and evaluation, for a designated group
• Manage a caseload of clients in the community and in secure or other provider settings, in and out of area
• Engage with secure services pathways and other providers within Cornwall and out of county to deliver care to people with complex needs, including mentally disordered offenders
• Deliver Tier 1-4 interventions as a team member under the SWPC regional model of care
• Act as Ministry of Justice social supervisor for identified clients
• Provide a consultation service for other mental health services
• Mentor and support learners and junior staff
• Travel frequently across the county and out of county, including to areas not reached by public transport
