Job summary

Cornerstone Community Care is seeking Complex Care Practitioners to support eight individuals with complex care needs who live in their own flats. You will collaborate with a multidisciplinary team, families and guardians to ensure robust, outcome-focused support plans are in place and followed.

Qualifications

• Experience in Health & Social Care preferred.
• Commitment to studying for SVQ SCQF level 7 and completing required training.
• Demonstrates resilience, transferable skills and a positive attitude.

Responsibilities

• Support individuals to manage their own homes and maintain independence.
• Develop and implement robust, outcome-focused support plans with multidisciplinary teams.
• Assist with social, health and wellbeing needs and encourage participation in meaningful activities.
