Job summary

Cornerstone in Scotland is seeking Complex Care Practitioners to join a friendly team supporting eight individuals in their own flats. Permanent roles with flexible hours between 16 and 39 per week, plus relief opportunities for flexibility.

You will work with families and guardians to ensure robust support plans, helping individuals manage homes, maintain independence, and access leisure activities in their local communities. Training and PVG/SVQ pathways are provided.

Qualifications

• Experience in Health & Social Care preferred or transferable skills with a positive attitude.
• Excellent communication, both verbal and written, with families and MDTs.
• Numeracy skills to maintain accurate records and charts.

We're on the look out for new colleagues to join our amazing #TeamBaxterView as Complex Care Practitioners on a permanent basis between 16 and 39 hours per week. We have relief opportunities too, if you're looking for something a bit more flexible.

Our team and the individuals you'll support

You'll become part of a friendly, dedicated welcoming team who provide support to eight individuals who have varying complex care needs to live in their own flats.

In addition to working directly with individuals, you'll also work in partnership with a multi-disciplinary team and the individual's families and guardians to ensure each individual has a robust and outcome focused support plan.

You'll support individuals to manage their own homes, increase and maintain their independence and encourage and support them with their social, health and well-being needs.

The people we support all have different hobbies and interests too, so we’ll need you to bring creativity and enthusiasm into supporting them to access leisure activities, engage in meaningful activities and take part in hobbies and interests that are important to them, both within their home and out in their local communities.

About You

Due to the needs of the individuals we support, we're ideally looking for people who have experience of providing care and support within the Health & Social Care sector, however, if you don't have direct care experience but can demonstrate transferable skills, resilience, and a positive attitude, we'd still love to hear from you

We will also need you to bring a commitment to studying for and achieving an SVQ (SCQF level 7) and completing training and further qualifications which are identified as essential for our service.

We will fund and support you with any training and qualifications you'll need and we will even reimburse your yearly SSSC registration and PVG fees!

To be a great Complex Care Practitioner, you'll need:

• A solid understanding of professional boundaries, accountabilities and responsibilities
• A passion for delivering excellent person-led care and support
• Excellent communication skills (verbal, listening and written)
• Numeracy skills to ensure accurate records are kept
• The ability and confidence to deal with behaviours that can challenge
• A caring and professional attitude and approach
• A positive approach to learning
• Confidence in the use of computers

It would be great if you also have:

• A full, clean driving license as some of the people we support have their own mobility cars and enjoy going out for drives (but this isn't a deal breaker for the right candidates).

Shift Patterns

Typically, shift patterns will fall between the hours of 0800-2200 hours, but can vary on occasion if the individuals we support have events planned that they'd like to attend. Weekends, public holidays and waking nights will also form part of your rota on a rotational basis. However, we appreciate the importance of a work life balance and offer flexibility where we can.

Successful candidates will be required to join the Protection of Vulnerable Groups Scheme and be committed to working towards an SVQ in Social Services and Healthcare with the support from our Training Academy.
