Job summary

Cornerstone in Scotland is seeking Complex Care Practitioners to join a friendly team supporting eight individuals in their own flats. Permanent roles with flexible hours between 16 and 39 per week, plus relief opportunities for flexibility.

You will work with families and guardians to ensure robust support plans, helping individuals manage homes, maintain independence, and access leisure activities in their local communities. Training and PVG/SVQ pathways are provided.

Qualifications

• Experience in Health & Social Care preferred or transferable skills with a positive attitude.
• Excellent communication, both verbal and written, with families and MDTs.
• Numeracy skills to maintain accurate records and charts.

Responsibilities

• Support individuals to manage their homes and maintain independence.
• Collaborate with multi-disciplinary teams and guardians to develop robust support plans.
• Help individuals access leisure activities and meaningful community involvement.
