Job summary

Nottinghamshire County Council is seeking a dedicated Multi-Skilled Community Care Officer to support adults with sensory loss within the ADVIS service. You will help individuals live independently and safely in their communities, combining social work and occupational therapy to achieve personalised outcomes.

Requirements include NVQ Level 3 or equivalent, knowledge of adult social care legislation, and at least two years’ relevant experience. Strong IT and record-keeping skills are essential.

Qualifications

• NVQ Level 3 health or social care qualification (or equivalent).
• Knowledge of community care services and adult social care legislation.
• Experience in a relevant setting for at least two years.
• Proficiency in record-keeping and IT applications.

Responsibilities

• Assess and review the needs of individuals and their carers.
• Coordinate support to achieve personalised outcomes.
• Promote independence and connect individuals to community resources.
• Provide practical support and communication assistance.
• Contribute to safeguarding and care planning.
• Collaborate with social workers, occupational therapists, and partner agencies.
• Use IT systems effectively for casework and reporting.
