Job summary

Nottinghamshire County Council is seeking a Community Care Officer to join the Adult Social Care team. You will assess needs, plan personalised support, and link people with community resources to promote independence.

You will work with Social Workers and health colleagues, handle a caseload, and contribute to safeguarding. A relevant health or social care qualification (NVQ Level 3 or equivalent) is required, plus IT skills and at least two years in care work.

Qualifications

• A relevant health or social care qualification (NVQ Level 3 or equivalent) or evidence of required skills and experience.
• Knowledge of community care services and legislation, including the Care Act 2014.
• At least two years of experience in care work (paid or voluntary).
• Proficiency in using IT applications for record-keeping and reporting.

Responsibilities

• Conduct assessments to identify the needs of individuals and their carers.
• Develop tailored support plans that encourage independence.
• Connect individuals with community resources and creative solutions.
• Manage your own caseload while collaborating with Social Workers and health colleagues.
• Contribute to safeguarding efforts and respond to urgent situations.
• Provide information, advice, and support to individuals and their families.
