Job summary

Nottinghamshire County Council is seeking enthusiastic Community Care Officers to join the Urgent and Emergency Care Service in the South of the county. The role focuses on helping individuals transition safely from hospital to home and living independently with support.

You will attend MDT meetings, conduct home assessments, support carers and contribute to duty rotas, with some weekend work. Hybrid working is available along with generous leave and a strong pension.

Qualifications

  • Relevant health or social care qualification (e.g., NVQ Level 3)
  • Knowledge of community care services and adult social care legislation
  • Proficiency in using IT applications for case management and reporting

Responsibilities

  • Attend Multi-Disciplinary Team (MDT) meetings to assist with discharge planning.
  • Conduct assessments and reviews in homes and care units, using strengths-based approaches.
  • Support carers and foster professional relationships.
  • Contribute to a duty rota, including some weekend work to meet service needs.
  • Collaborate with colleagues and managers to achieve timely outcomes.
