Job summary

NHS Scotland's Community Rehabilitation Team in Levenmouth is seeking an experienced RSW to deliver therapeutic treatment programmes to adults in the ward setting or in their own homes. You will work with Occupational Therapy, Physiotherapy and District Nursing to prevent hospital admission and support early discharge, with duties across 7 days and 25 hours per week.

The role requires SVQ Level III in Health and Social Care (or equivalent), and experience as a support worker is desirable.

Qualifications

• SVQ Level III in Health and Social Care or equivalent.

Responsibilities

• Deliver therapeutic treatment programmes to service users in the ward setting or in their own homes.
• Work with Occupational Therapy, Physiotherapy and District Nursing Services to support intermediate care.
• Carry out ongoing treatment programmes for a mixed caseload, mainly older people.
• Visit patients in their own homes and deliver equipment where needed.
• Work flexibly across 7 days, including 24.7 hour coverage.
