Job summary

NHS National Services Scotland is seeking an experienced Registered Social Worker (RSW) to join the Community Rehabilitation Team in the Levenmouth locality. The role involves delivering therapeutic activities to adults in hospital, clinic, or home settings, in collaboration with OT, PT and District Nursing colleagues.

Flexible 24/7 community hours are required to cover patient visits and equipment delivery.

Qualifications

• SVQ Level III in Health and Social Care or equivalent.
• Experience as a support worker desirable.
• Experience with older people is desirable.

Responsibilities

• Deliver therapeutic treatment programmes with Occupational Therapy, Physiotherapy and District Nursing in the ward setting or patients' homes.
• Carry out ongoing treatment programmes for a mixed caseload, primarily older people.
• Work as part of a multidisciplinary team and be flexible to work across locations and sites.
• Visit patients at home and assist with equipment delivery as needed.
