The post supports District Nursing Service delivering care in patients' homes and residential settings, across Stockport, working with community teams and GPs to provide high-quality care.

You should have strong assessment and organisational skills, be willing to travel efficiently across multiple community sites between 08:00 and 18:00, seven days a week.
