The post requires a registered nurse with at least two years' RN experience; post-registration community experience is desirable. Service hours are 8am–8pm, seven days a week, with flexible shifts to meet service needs.

Based in Gateshead, the Locality Team works across multiple settings to provide safe, effective patient care and ongoing professional development.

Qualifications

• Must be a registered nurse with current NMC registration.
• Minimum 2 years RN experience required.
• Community nurse experience desirable.

Responsibilities

• Provide high quality patient care in the community under supervision of a senior clinician.
• Deliver care to patients in various community settings.
• Maintain standards of care and evaluate evidence-based practice.
• Delegate tasks appropriately ensuring patient safety in the community.
• Support development of other staff and service provision.
• Assess health needs of patients and carers in their homes.
• Treat diseases per medical protocols under supervision.
• Plan and evaluate nursing care for individual patients.
• Reflect on clinical practice to support ongoing professional development.
