Job summary

Derbyshire Community Health Services NHSFT seeks a qualified nurse to join the Staffa and Limes District Nursing Team on a 12-month fixed-term basis. The role involves delivering high-quality community care, travelling across Derbyshire to visit patients at home and support hospital discharge.

The post is Band 5 with the Agenda for Change pay scale (£32,073–£39,043 per year, pro rata). It offers full-time hours, flexible working where possible, and access to training, development and NHS

Qualifications

• NMC registered nurse with current UK registration.
• Evidence of recent relevant training.
• Experience of working in the community.
• Experience with long-term conditions.
• IT skills.
• Willingness to travel across Derbyshire.

Responsibilities

• Provide high-quality nursing care in patients' homes to keep them safe at home.
• Assess, plan, implement and evaluate personalised patient care.
• Support District Nurse and junior staff in delivering high-quality care.
• Collaborate with Integrated Community Teams and GP surgeries.
