Bush & Company Rehabilitation Limited is seeking a Clinical Case Manager to work flexibly within a specialist rehabilitation and case management setting. The ideal candidate is an experienced clinical professional, keen to improve outcomes for individuals following significant injuries.

You will conduct assessments, develop rehabilitation plans, and liaise with clients and multidisciplinary teams. This role requires strong clinical reasoning, autonomous decision-making, and excellent communication skills, with opportunities for clinical supervision and peer support.

Qualifications

• 5+ years post-qualification clinical experience.
• Current registration with a relevant professional body.
• Experience in establishing or implementing rehabilitation packages.

Responsibilities

• Conduct assessments within home or community settings.
• Develop and review rehabilitation recommendations.
• Coordinate appropriate rehabilitation services.
