Job summary

Frimley Health NHS Foundation Trust is seeking an adaptable Bank CHC Clinical Case Manager on a flexible basis. The role delivers senior clinical leadership, manages complex CHC pathways, and contributes to service improvement within the CHC National Framework.

The successful candidate will oversee CHC assessments and reviews, Fast Track pathways, care planning and commissioning, and retrospective reviews, while modelling compassionate, person-centred care and maintaining robust governance.

Qualifications

• Current professional registration with a regulatory body.
• Enhanced DBS check completed.

Responsibilities

• Provide senior clinical leadership and manage complex pathways under the CHC National Framework.
• Conduct CHC assessments and reviews, Fast Track pathways, care planning and commissioning.
• Lead on complex case management, appeals and retrospective reviews.
• Support service improvement, governance and performance within the National Framework.
• Contribute to resource, business and financial planning; provide leadership through staff induction and multidisciplinary teamwork.
