Job summary

Milton Keynes City Council is recruiting Support Workers to join our Adult Provider Services team. You will help adults with dementia, learning disabilities and complex health needs live with dignity and meaning, across the community and in people’s homes. This is a site-based role with flexible patterns to meet client needs.

This role requires a full UK driving licence and access to a vehicle; you may be asked to provide evidence of your licence and business-use insurance.

Qualifications

• Experience and understanding of delivering care and support to adults with diverse needs.
• Clear and respectful communication to colleagues, families and service users.
• Understanding and practice of confidentiality and data protection.

Responsibilities

• Deliver personalised care and support for people with a range of individual needs.
• Follow individual care plans and report changes in a person's needs, health or circumstances.
• Plan and support meaningful activities that reflect each person's choices, needs and preferences.
• Provide personal care while protecting each person's privacy, dignity and wellbeing.
• Keep clear and accurate written and electronic records, and communicate effectively with families, colleagues and external organisations.
• Recognise and respond to risks or crisis situations, seeking support and guidance when needed.
