Job summary

Milton Keynes City Council is recruiting Support Workers to join our Adult Provider Services team. You’ll deliver personalised care and practical support for adults with dementia, learning disabilities and complex health needs, helping them live with dignity and participate in meaningful activities.

The role is site-based with flexible patterns, and preparation is underway for a move to Linford Wood Place to support high-quality care and outcomes.

Qualifications

• Understanding of the needs of people who require care and support.
• Clear and respectful communication skills.
• Confidentiality is important when recording information.

Responsibilities

• Deliver personalised care and support for people with a range of individual needs.
• Follow individual care plans and report changes in a person’s needs, health or circumstances.
• Plan and support meaningful activities that reflect each person’s choices, needs and preferences.
• Provide personal care while protecting each person’s privacy, dignity and wellbeing.
• Keep clear and accurate written and electronic records, and communicate effectively with families, colleagues and external organisations.
