Job summary

Voyage Care is seeking Fieldcare Supervisors to lead our supported living services in Bridgwater and Taunton. This pivotal role involves guiding a team to deliver person-centred care and fostering independence for the people we support.

You will develop and implement care plans, conduct assessments, induct new staff, complete supervision and medication audits, and attend multi-disciplinary meetings. On-call duties are included to support service needs.

Qualifications

• Experience in a care management role and Level 3 Health & Social Care.
• Experience working with autism, brain injury or complex needs.
• Ability to develop person-centred support plans and conduct assessments.

Responsibilities

• Lead a team to deliver supported living services in Bridgwater/Taunton.
• Develop and implement person-centred support plans and conduct needs assessments.
• Induct new colleagues, conduct supervisions, and support medication competencies/audits.
• Attend multi-disciplinary meetings and prepare reports; assist Branch Manager.
• Take part in on-call rota and ensure high quality care outcomes.

Fieldcare Supervisors with a passion for enhancing the quality of life of the people we support are at the heart of our community service teams. We’re currently searching for a fantastic Field Support Supervisors to join us in leading our supported living services in Bridgwater and Taunton. This service is a welcoming and friendly environment with a dynamic team that supports a fantastic group of people!

Our Fieldcare Supervisors will be responsible for managing a team of colleagues with the purpose of helping people to gain more independence in their daily lives. You could be developing and implementing person centred support plans, conducting care needs assessments, inducting new colleagues, completing supervisions/appraisals, completing medication competencies and medication audits, attending mutli-disciplinary meetings, completing reports and generally assisting the Branch Manager in the day-to-day management of our services. You’ll never lose sight of why you choose to do this, as our Field Support Supervisors regularly visit their services and spend time with the people we support.

You will also take part in the on-call rota.

This is why at this service our Fieldcare Supervisors should have previous experience in a care management role and a Level 3 in Health & Social Care as well as experience working with individuals with autism, acquired brain injury, complex individuals with mental health needs, traumatic backgrounds and challenging behaviours.
