Job summary

A leading senior care provider in Ealing is seeking a dedicated care coordinator to support care consultations, maintain accurate records, and build relationships with clients and their families. The ideal candidate will have a Level 3 NVQ in Health and Social Care, experience in the care sector, and excellent communication skills. This role is crucial for ensuring high-quality care delivery and supporting a growing team. A full driving license is required as visits to client's homes are necessary.

Qualifications

  • Experience in the care sector delivering personal care services.
  • Experience of working with MAR sheets and medication in a supervisory role.
  • Good working knowledge of IT systems and virtual communication platforms.
  • Excellent communication and relationship-building skills.
  • Confidentiality and knowledge of health and social care regulations.
  • Confidence in using care management technology and IT systems.

Responsibilities

  • Support care consultations and service reviews.
  • Maintain accurate client and Care Professional records.
  • Build positive relationships with clients and their families.
  • Maintain accurate client and Care Professional records on Home Instead software and People Planner.
  • Conduct client and Care Professional introductions.
  • Build and maintain positive relationships with clients, their families and professionals.
  • Carry out client reviews and action these accordingly.
  • Support and mentor Care Professionals.
  • Provide support to the Client Experience team.
  • Maintain regular contact with Clients and Care Professionals.
  • Participate in on-call duties as required.
  • Ensure compliance with Equality, Diversity and Equal Opportunities Policy.

Job description

Our office was established in 2016and our mission is to brighten the lives of seniors giving them a sense of purpose, wellbeing & worth. This role will play a vital part ensuring that our clients receive the best in class quality care and support the growth of our business.

Job Description
  • Support care consultations, service reviews and completing client support plans.
  • Support with staff supervisions and client QA’s.
  • Create, update and audit client care plans and assist with digital care planning.
  • Maintain accurate client and Care Professional records on Home Instead software and People Planner.
  • Conduct client and Care Professional introductions.
  • Build and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care.
  • Carry out client reviews and action these accordingly.
  • Support and mentor Care Professionals.
  • Provide support to the Client Experience team.
  • Maintain regular contact with Clients and Care Professionals.
  • Any other duties as directed by your line manager.
  • Participate in on-call duties as required.
  • Ensure compliance with Home Instead’s Equality, Diversity and Equal Opportunities Policy, in respect of employment and service delivery.
Qualifications
  • Level 3 NVQ in Health and Social Care or equivalent.
  • Experience in the care sector delivering a wide range of personal care services.
  • Experience of working with MAR sheets and medication in a supervisory role.
  • Passionfor delivering the highest quality care to our clients and helping them to live independently and happily in their own homes.
  • Knowledge and understanding of legislation and regulations specific to Health and Social Care.
  • Good communication skills with the ability to build rapport quickly.
  • Must be confident to use care management technology including providing support and training to Care Professionals.
  • Must understand the importance of confidentiality working within current legislation.
  • Good working knowledge of IT systems with experience of Microsoft Office or Google Suite and virtual communication platforms with the aptitude to learn and adopt new technologies and software where appropriate.
  • Must have full driving license and means of transport if required within the territory to visit clients.
  • Be organised and flexible to meet the needs of the business.
