Job summary

Caremark Pulborough and Horsham is seeking a proactive Team Leader to supervise care delivery across Pulborough and Horsham. You will lead field-based teams, conduct reviews, and assess needs to create tailored care plans that reflect each individual’s preferences and requirements.

The role focuses on mentoring Care Assistants, ensuring regulatory compliance such as CQC standards, and maintaining documentation.

Qualifications

• At least 2 years’ experience in a care setting (home care preferred)
• Minimum of Level 3 Health & Social Care qualification
• A full UK driving licence
• Participate in the out-of-hours on-call rota to respond to emergencies and ensure service continuity
• Strong written and verbal communication skills
• Compassionate and calm under pressure
• Confident using Microsoft Office, care planning and other systems (Access/People Planner)
• Knowledge of CQC standards and regulatory compliance
• Knowledge of safeguarding protocols and incident reporting procedures.

Responsibilities

• Lead frontline care teams, conduct care reviews and risk assessments to create tailored plans.
• Mentor Care Assistants, observe care delivery and ensure safety and quality across Pulborough and Horsham.
• Coordinate with office staff, manage documentation and support carers during emergencies or urgent visits.
