Job summary

Powys County Council is seeking Enablement and Care Support Workers in Welshpool. Roles involve implementing Personal Plans to help people reach their maximum potential and supporting individuals in the community with care, safety, and independence.

Responsibilities include assisting with personal care, medication, and manual handling within a person-centred approach, while complying with Care Inspectorate for Wales requirements.

Responsibilities

• To implement Personal Plans to enable people to reach or regain their maximum potential in the community.
• To support people to meet identified assessed needs in a person-centred way as per the personal plan.
• To support people with personal care, manual handling and medication in line with the plan and medication policy.
• To help people reconnect with local communities and networks and maximise independence.
• To understand what matters to a person and help manage risks to maximise choice and control.
