Job summary

Powys County Council in Welshpool (Y Trallwng) is recruiting an Enablement and Care Support Worker for a permanent, on-site role. The post focuses on implementing Personal Plans to help people reach their maximum potential and supporting daily living within the community with independence and safety in their own environment.

You will assist people with complex needs in personal care, manual handling and medication in line with personal plans and policy.

Responsibilities

• Implementing Personal Plans to enable people to reach or regain their maximum potential based on assets, resources and abilities.
• Assist or enable service users to remain living within the community, enhancing quality of life and maintaining safety in their own environment.
• Comply with requirements of a service registered with the Care Inspectorate for Wales under the Registration and Inspection of Social Care Act 2016.
• Support people to meet identified assessed needs and outcomes in a person-centred way as detailed within a personal plan.
• Deliver support in a variety of settings, maximising independence as much as possible.
• Support people with complex needs in personal care, manual handling and medication in line with personal plans and medication policy.
• This position has a requirement for an Enhanced DBS Check.
