Job summary

Target Housing Ltd. is seeking a Housing Support Worker for the THRIVE Project in Sheffield to help individuals with complex needs build independence and stability.

You will provide tailored support, create risk and support plans, and empower clients to sustain tenancies through a structured, person-centred approach. Full UK driving licence is essential; mileage may be reimbursed and a strong emphasis on trauma-informed and PIE approaches is expected.

Qualifications

• Experience supporting individuals with multiple complex needs.
• Ability to engage hard-to-reach clients and build trust.
• Knowledge of risk assessment and support planning processes.
• IT literacy for case recording and reporting.

Responsibilities

• Deliver 1:1 and group support using a case management approach.
• Complete risk assessments, support plans, and safety plans.
• Motivate clients to achieve personal goals and independence.
• Design and deliver activity programmes focused on independence and wellbeing.
• Maintain accurate records in line with data protection standards.
• Build partnerships with external agencies to provide holistic support.
