Job summary

5 Star Recruitment is seeking a qualified Social Worker to deliver a high quality adult social care service in Wales. You will carry a caseload of adults with disabilities, assess needs, develop care plans using person-centred planning and involve citizens, carers and families in decisions that affect their lives.

You will work with colleagues across health, social services and the voluntary sector to provide integrated support, ensure safeguarding, maintain records, meet statutory timescales,

Qualifications

• Recognised Social Work qualification.
• Registered with Social Care Wales.
• CPEL consolidation award.
• Evidence of post-qualification learning or achievements (e.g. Approved Mental Health Practitioner, Best Interest Assessor, Experienced Practitioner).
• Knowledge of Social Services and Wellbeing Act 2016.
• Knowledge of Person Centred Planning approaches.
• Ability to work with service users to develop appropriate, effective and sustainable responses to meeting need using a range of resources available.
• Experience of complex case work.
• Experience of safeguarding procedures.
• Ability to write accurate, coherent reports and records.

Responsibilities

• Provide a direct social work service to Adults with Disabilities in accordance with statute, national and regional policies.
• Work in partnership and develop effective relationships with colleagues in other Social Services areas and health to deliver an integrated response.
• Build effective relationships with citizens and their families to identify and meet needs and safeguard rights.
• Ensure that statutory timescales for assessment and review are met.
• Use Person Centred Planning approaches to support participation in assessment, care planning and review.
• Respond to safeguarding concerns following relevant policies.
• Carry out safeguarding investigations and complete risk assessments.
• Contribute to reports and plans at Strategy meetings and reviews.
• Maintain professional accountability for resource allocation and administration.
• Maintain good recording practices and ensure timely completion of records.
