Job summary

Vale of Glamorgan Council is seeking an Adults Social Worker to join the Long Term Care service. You will assess citizens under statutory duties, plan and commission services, and monitor care plans in partnership with citizens and carers.

You will work with other professionals and partners to secure resources, manage a caseload, keep electronic records, and participate in supervision and ongoing professional development to maintain Social Care Wales registration.

Responsibilities

• Undertake assessment of citizens in accordance with statutory duties under relevant legislation.
• Plan and commission services to enable adults with care & support needs and carers to overcome barriers to achieving personal outcomes, and monitor and review care plans through co‑production with citizens and carers.
• Work in partnership with citizens with care and support needs, carers, and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them.
• Negotiate and co‑ordinate applications for resources and work in partnership with internal and external partners, both statutory and voluntary, on behalf of citizens and carers.
• Undertake case management responsibility for a specific caseload and prioritise workload demands appropriately.
• Maintain up to date electronic case files and relevant documents, providing accurate records relating to performance management and other statistical data as required.
• Prepare for and participate in supervision and Personal Development Review (#Its About me) sessions with supervisor/line manager, and identify personal development and training needs to maintain Registration with Social Care Wales.
• Undertake training to support continuing professional development to ensure that the Council is able to retain the provision of its statutory functions, including a willingness for staff to undertake further training to become Approved Mental Health Practitioners, or other more specific, senior roles.
• Supervise or mentor other staff or students where appropriate, and offer input into training programmes and Team Practice Issues meetings where appropriate.
• Take part in office duty rotas and other activities associated with a multi‑disciplinary team.
• Ensure compliance with the Council's Financial Regulations, Standing Orders, Policies and Procedures.
• Implement the principles of the Council's Equal Opportunity Policy while carrying out the above duties.
