Job summary

5 Star Recruitment is seeking a Social Worker to support adults and carers within the Care Act framework, delivering person-centred Social Care and safeguarding. The role emphasizes assessment, planning, and multi-disciplinary collaboration to promote independence and wellbeing.

You'll work with MDTs, complete safeguarding enquiries, organise strategy meetings, and advocate for rights and entitlements, while logging care plans and maintaining professional standards.

Qualifications

• Social Work qualification with registration in good standing.
• Evidence of continuing professional development (CPD) in line with PCF.
• Experience of safeguarding and working with adults and carers.

Responsibilities

• Carry out needs-led assessments, care planning and reviews, initiating multi-disciplinary assessments as required.
• Provide social work input to adults and carers, promoting self-determination and autonomy.
• Submit funding applications and liaise with local commissioners where appropriate.
• Organise and chair multi-disciplinary and strategy meetings and professional case conferences.
• Communicate complex assessment information effectively to stakeholders and maintain records within timescales.
