Job summary

5 Star Recruitment is seeking a qualified social worker in Wales to undertake assessments under the Social Services and Well-being Act, with a focus on outcomes and co-production with service users and carers. You will prepare care plans, collaborate with multi-disciplinary teams, and ensure accurate records and reporting.

Responsibilities include planning, reviewing, and coordinating resources to meet identified outcomes, while safeguarding vulnerable children and adults and staying up to date

Qualifications

• Professional Social Work qualification i.e. MA/Degree in Social Work or recognised predecessor equivalent (CQSW, DipSW, CSS).
• Knowledge of the Social Services and Well-being Act (Wales) 2014.
• Knowledge of the Mental Capacity Act and the Mental Health Act.

Responsibilities

• Undertaking assessments in line with the Social Services and Well-being Act (Wales) 2014 for children, adults and carers.
• Identify outcomes and develop care and support plans with monitoring and reviews.
• Plan and manage care and resources within available budgets.
• Engage in inter-agency collaboration and maintain electronic records in line with policy.
• Safeguard and promote the wellbeing of vulnerable individuals and contribute to service development.

Education

MA / Degree in Social Work or recognised predecessor (CQSW, DipSW, CSS)
