Thrive in Slough is seeking a dedicated Young Person’s Adviser to support care-experienced young people aged 18 to 25. You will manage a caseload, attend Care Plan Reviews and help develop Pathway Plans to promote independence and opportunities in education, employment and personal growth.

You will work on a hybrid basis (minimum 3 days in the office), liaising with schools, housing providers and health services, while maintaining accurate case records and ensuring timely statutory duties are

Qualifications

• Must possess a relevant qualification and experience working with young people in a social care or support setting.
• Knowledge of legislation and entitlements relevant to care leavers is advantageous.

Responsabilités

• Manage a caseload of care leavers and meet statutory duties within required timescales.
• Attend Care Plan Reviews and co-develop Pathway Plans for each young person.
• Maintain regular contact with care leavers, including those who prefer remote support.
• Promote educational attainment and liaise with schools, colleges and universities.
• Support care leavers in securing and sustaining suitable accommodation using Leaving Care Grant.
• Maintain accurate and timely case records in line with statutory and departmental requirements.
