The Methodist Church in Britain is seeking an adviser to support care-experienced young people aged 18 to 25 in Slough. This permanent role sits in the Children in our Care Team, handling caseloads, meeting deadlines and coordinating with housing, health, and other agencies on Pathway Plans.

The post requires a relevant qualification and social care experience, with a hybrid working pattern including three days in the office and occasional evening/weekend work.

• Manage a caseload of care leavers, ensuring statutory duties are met within required timescales.
• Work collaboratively with housing providers, health services and other agencies for holistic support.
• Attend Care Plan Reviews and co-develop Pathway Plans tailored to each young person's needs.
• Maintain regular contact with care leavers, including those who prefer remote support.
• Promote educational attainment, liaising with schools, colleges and universities.
• Support care leavers to secure and sustain suitable accommodation using the Leaving Care Grant.
• Maintain accurate and timely case records in line with statutory and departmental requirements.
• Promote opportunities for education, employment and personal growth.
• Work flexibly, including evenings and weekends.

This permanent post sits in the Children in our Care Team at Slough Children First, supporting care-experienced young people aged 18 to 25 into adulthood.
