Job summary

Reading Borough Council is seeking a dedicated Personal Advisor to join the Leaving Care Service. You will work with care-experienced young people aged 18-25 to support their progression into adulthood, focusing on education, employment, health, housing and independent living.

You will manage a caseload, deliver direct interventions, and collaborate with partner agencies to remove barriers and build resilience.

Qualifications

• Experience working with care-experienced young people.
• Ability to advocate and empower young people in decision-making.
• Experience coordinating multi-agency support and safeguarding.

Responsibilities

• Manage a caseload of care-experienced young people aged 18-25.
• Provide secondary support to 16-18-year-olds for pathway planning and independence.
• Complete and review Pathway Plans identifying strengths, needs, goals and supports.
• Deliver direct interventions to promote independence, confidence and life skills.
• Support education, training, employment, housing and community engagement opportunities.
• Undertake risk assessments and respond to safeguarding concerns.
• Develop relationships with families, carers, partner agencies and community organisations.
